Up to 45 mins - $300 (Psychiatrist); $235 (Nurse Practitioners)
Up to 60 mins - $450 (Psychiatrist); $335 (Nurse Practitioners)

INSURANCE
Please note that FCDC is considered an "out of network provider" and does not participate with any insurance companies. At the end of each appointment, you will be provided with a receipt that can be submitted to your insurance company for reimbursement if you have out of network benefits. Please call your insurance company to inquire about your benefits as every insurance company is different and has different deductibles, reimbursement rates, and procedures for submitting claims. If you have any additional questions or concerns, please feel free to contact us and our support staff would be happy to assist you.

Good Faith Estimate
As of January 2022, You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the new law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
