Medication Monitoring

Initial Psychiatric Evaluations

Scheduled with a Psychiatrist -$500

Scheduled with a Nurse Practitioner - $400

Follow Up Appointments:

Up to 30 mins - $250 (Psychiatrist); $185 (Nurse Practitioners)

Up to 45 mins - $300 (Psychiatrist); $235 (Nurse Practitioners)

Up to 60 mins - $450 (Psychiatrist); $335 (Nurse Practitioners)

*Special Requests will be charged an additional $50 fee.

Specialty Services

Medical Marijuana Evaluations

Evaluation for new patients - $750

Evaluation for current patients - $400

ADOS Testing

Evaluation for new patients - $850

Evaluation for current patients - $650

INSURANCE

Please note that FCDC accepts Aetna, Cigna and Carelon insurances. Any other insurances are considered "out of network". For out of network insurances, a receipt will be provided at the end of your appointment that can be submitted to your insurance company for reimbursement, if you have out of network benefits. Please call your insurance company to inquire about your benefits as every insurance company is different and has different deductibles, reimbursement rates, and procedures for submitting claims. Good Faith Estimate

As of January 2022, You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the new law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
